Registered Nurse Case Manager (RNCM)

The Hospice nurse is responsible for the care of hospice patients, completes admissions, and intermittent call duties. Characteristics of the optimal candidate include strong clinical skills, computer skills, excellent communication skills, flexibility, ability to work well within a team, and ability to work autonomously.


Certified Nursing Assistant (CNA)

Certified Nursing Assistants provide personal care service to the terminally ill patient in accordance with the plan of care as assigned and delegated by the Registered Nurse. Characteristics of the optimal candidate include strong CNA skills, computer skills, excellent communication skills, flexibility, ability to work well within a team, and ability to work autonomously.


Social Worker (SW)

Social Workers provide supportive and medical social services to terminally ill patients and their families in accordance with an appropriate plan of care. The Social Worker will assume responsibility for the development, implementation, and management of psycho-social services provided to hospice patients and families in the home care or residential setting. Characteristics of the optimal candidate include strong clinical skills, computer skills, excellent communication skills, flexibility, ability to work well within a team, and ability to work autonomously.

Chaplain

Chaplains provide direct spiritual/pastoral/bereavement support and/or counseling to patients/families, hospice staff, and volunteers as needed, acknowledging and respecting their beliefs, culture and values related to life’s meaning including suffering, loss and a desire to receive support. Characteristics of the optimal candidate include Demonstrates mastery of job, computer skills, excellent communication skills, flexibility, ability to work well within a team, and ability to work autonomously.


Volunteer

Volunteers are the heart of Front Range Hospice. They work as a part of the palliative care team to provide a special kind of compassion for our patients and their families. Whether it is offering a unique presence, offering a listening ear, providing grief support, or supporting the administrative tasks of our hospice, the time that our volunteers give to Front Range Hospice allow our hospice to provide exceptional care to our patients and their families.
